There are children who, due to their dramatic health situation, are defined as "incompatible with life".
And instead they live. And they want to live, if possible, in their own home.

There are children whose health conditions would have been incompatible with life until a few years ago but who today, thanks to medical advances, live to adulthood. Children with disabilities of varying severity can live, with quality care, in their own home.

What are they

Home care includes integrated health and social-health activities that are provided directly to the home of the assisted minor.

TYPE OF HOME SERVICES PROVIDED

Health area

• nursing, specialist medical, psychological assistance, complex pathologies
• functional rehabilitation (neuropsychomotor skills, physiotherapy, speech therapy, occupational therapy)

Auxiliary area

• assistance in mobilization and personal hygiene
• assistance with activities of daily living

Who are they addressing?

Home care is aimed at minors suffering from serious pathologies: chronic/degenerative, neurological, oncological, genetic syndromes< /strong>, serious prematurities or to minors who require specific therapies.

 

They are, therefore, entitled to home care for all minors (from 0 to 18 years of age) with difficulty accessing outpatient clinics or other local facilities. There are no age or income limits.

Delivery method

Home care (C-DOM) can be provided both in agreement with the Regional Health System (SSR) and privately.

The Maddalena Grassi Foundation is one of the few organizations that operates in Milanwhich is dedicated to home care for minors.

Activation of the service

Prescription
Home care can be prescribed by:
  • the general practitioner
  • the freely chosen pediatrician
  • the hospital specialist for protected discharges
  • the territorial socio-healthcare UdO (the set of territorial or home, day and residential structures that make up the network of social-welfare services in the area).
Attention: it is necessary for there to be a person (care giver) in the home dedicated to caring for the patient. The caregiver is a family member who experiences the treatment process together with the patient. He or she plays an informal role of assistance, support and closeness, actively participating in the experience of the disease and dedicating himself or herself to the daily activities of caring for the person.

If you or a family member has been prescribed to activate the home care service (C-DOM), what you need to do isto contact your reference ASST.

After an evaluation, the ASST will confirm the indication to the C-DOM service and establish the Intervention Project (P.I.) and will assign the appropriate treatment profile, issuing the relevant health voucher and providing a list of subjects accredited to the provision of the service.

Once the Maddalena Grassi Foundation has been contacted, our professionals will carry out the first evaluation visit and define the Service Plan Individual Assistance (P.A.I.) by activating taking charge.

(The Foundation is responsible for the healthcare material necessary for the execution of the services provided for by the plan and the service is totally free for the user).

The Territorial Socio-Health Companies (ASST) participate together with the other providers, both public and private, accredited with the national health system of the C-DOM service (as foreseen by the Essential Levels of Assistance, LEA) and any additional levels defined by the Region with its own resources, in the logic of taking charge of the person, defining an Individual Plan (PI).
